Procedure for Filing and Verifying Applications for an Excerpt

 Filing of applications

 

The Criminal Register of the Czech Republic issues an excerpt from the Criminal Register on the basis of the application of a person, whose identity was verified. An excerpt from the Criminal Register of the Czech Republic (hereinafter referred as ,,an excerpt”) is issued also to an agent of the person on the basis of a officially authenticated power of attorney.

 

The application for an excerpt can be filed: 

 

-In the seat of the Criminal Register of the Czech Republic at Praha 4, Soudní 1 (underground station Pražského povstání, line C). Internet address: http://portal.justice.cz.

- In a contact point of the CzechPOINT system (local and municipal authority, branch office of the CzechPost and the Czech chamber of commerce, notary). Addresses and business hours of contact points are presented on: http://www.czechpoint.cz/.
Attention:
Excerpts issued in contact point
of the CzechPOINT system are primary designed for usage in the territory of the Czech Republic.
For usage abroad is necessary to obtain an excerpt issued by the 
Criminal Register of the Czech Republic itself.

- In every local and municipal authority. Addresses and business hours of local and municipal authorities are presented on: http://portal.gov.cz

 

- Abroad in Czech embassies.

An administrative fee for receipt an application for an excerpt is 100,-CZK (In the seat of the Criminal Register only in cash).
For filing of application for an excerpt in Czech embassies abroad is an administrative fee 200,-CZK.

 

 

Verification of applications

 

In order to filing of application for an excerpt, the Criminal register of the Czech Republic verify an identity of applicant and a correctness of information presented in a written application.

 

a)    An applicant's identity is verified according to an original and valid personal identity card (a citizen’s identity card, a passport, a foreigner's residence permit, etc.), which includes a photograph of the applicant. In the event if another person files an application for an applicant, this person must provide an officially authenticated power of attorney, including its sworn translation into the Czech language.

b)    The correctness of the information presented in an application is verified according to an original and valid personal identity card (e.g. a citizen’s identity card - ID card) containing at least the name, current surname, surname at birth, date and place of birth, birth number and state citizenship of the applicant. Should any data be missing from a personal identity card (passport, foreigner's residence permit,  etc.), the applicant or their proxy submits their birth certificate, birth and baptism certificate, confirmation of birth, marriage certificate, etc.  Citizens of other states are to submit original documents or officially authenticated copies, including their sworn translations into the Czech language.
